Mental Health Case Manager, Psychiatry/Behavioral Science (Partial remote)
Minimum Qualifications:
Bachelor's degree in Psychology, Social Work or Mental Health / Behavioral Science. Criminal Justice degree considered with 15 hours of coursework in psychology, social work and/or counseling with 2 years of MH case management experience.
Job Summary:
This position is responsible for providing case management, social and habilitative services, and counseling to patients. Works under the supervision of a Psychologist, Mental Health Manager or Program Director.
Job Duties:
- Collaborate with program clinicians to coordinate ongoing care for TCHATT patients
- Act as liaison between patients and clinicians
- Disposition TCHATT cases upon discharge from services
- Develop and coordinate care plans for patients
- Assist with development and maintenance of the mental health resource database
- Demonstrate competency and skill in communicating with patients, families, and school personnel
- Participate in team meetings and provide status updates to team members and leadership
- Participate in conferences, workshops, and trainings related to your role
- Assist with maintaining project data and required reports
- Other duties as assigned
Preferred Qualifications:
- 1 year experience working with families.
- Must have proficient computer skills.

Actual salary commensurate with experience.
