Mental Health Case Manager, Psychiatry/Behavioral Science (Hybrid)
Minimum Qualifications:
Bachelor's degree in Psychology, Social Work or Mental Health / Behavioral Science. Criminal Justice degree considered with 15 hours of coursework in psychology, social work and/or counseling with 2 years of MH case management experience
Job Description:
This position is responsible for providing case management, social and habilitative services, and counseling to patients. Works under the supervision of a Psychologist, Mental Health Manager or Program Director.
Job Duties:
- Triage calls received by UTMB’s CPAN team.
- Fulfill resource requests.
- Refer calls to providers for consultation.
- Enroll clinics and providers in CPAN and PeriPAN.
- Contribute to the resource database for all projects.
- Liaisons with patient, patient families, on-site departments and community agencies to coordinate current treatment and/or assist with aftercare planning.
- Create and develop mental health education materials as assigned.
- Attend project related meetings and participate in project related activities.
- Adheres to internal controls and reporting structure.
Marginal or Periodic Functions:
- Performs related duties as required.
Salary Range:
- Commensurate with experience.
